Your views are very important to us.
We really aim to hear everything you have to say about your physical activity and wellbeing, so we have extended the deadline to give you an extra two weeks to have your say.
NEW DEADLINE: Monday 12th December.
If you are over 16 and live, work, study or regularly visit the City of Westminster, we really want to understand how you engage in play, physical activity, sport and leisure services and the key challenges you are facing.
We want to hear from you as your views will provide valuable insights that will help us shape and improve a range of community services.
The questionnaire takes around 15 minutes, and you will get the chance to enter the prize draw to win one of the following prizes:- 3 x £50 Amazon voucher
- 2 x 3-month free membership or
- 2 x 1-month free membership
The winners of the prize draw will be notified by 15 December, in time to do some shopping before the holidays or start the new year’s resolution of being more active early!
